Dental Insurance

The financial obligation for dental treatment is between you and this office regardless of your insurance coverage.  Payment is expected at each appointment for the treatment received that day.  We do accept VISA and MasterCard for your convenience.

We are more than happy to assist you in submitting your claims.  The information on our quick claim receipt is sufficient to file a claim with your insurance company for exams, hygiene appointments and some other less complex treatments. 

For more comprehensive procedures, we recommend submitting a predetermination of benefits while initial periodontal treatment begins.  Insurance companies require x-rays and other data to support a claim.  A pre-estimate not only informs you of what benefits are available, it also saves us from repeatedly sending x-rays, etc., as treatment progresses. 

Though we do not know every detail of each insurance plan, from experience we know that most insurance plans do not pay 100%, and usually there are limitations to the yearly benefits and exclusions to the contract. Ultimately, you are obligated to pay for treatment partially covered or not covered by your dental insurance plans.
